1. Renewal Form
Download the application at the bottom of the page. Complete it in full. If something does not apply to you just put N/A. Once your application is complete you can submit it via email or mail along with all of your other supporting documents.
Supporting Documents:
2. Proof of First Aid & CPR (if expired)
Submit proof of your current First Aid Training with a minimum of Standard First Aid and CPR (Level C). This certification can not be expired and must be current.
3. Submit Police Record Check (every three years )
Submit a current Criminal Record and Vulnerable Sector Check if it has been 3 years since your most recent submission.
4. Disclose Any Proceedings
Disclose any criminal investigations, criminal convictions or any professional misconduct proceedings/findings against you.
5. Pay Membership Fees
Pay all membership fees upon approval of application without delay.
Once your application has been processed and approved you will receive an online invoice which you can pay with a credit card. You can also pay by e-transfer to mail@ocdo.ca .
The 2025 fee is $450.
